It has not been a good month for the South Central Ambulance Service.
We reported how an ambulance called to Grove, near Wantage, was misdirected to Grove in Bedfordshire.
Now an inquest hears details of an earlier incident in which charity cyclist Patrick Royle waited an hour for an ambulance.
He died after an operator mistakenly listed the call as a non-emergency.
The inquest heard Mr Royle could have survived had he received immediate treatment.
What is also certain is confidence in our new regional ambulance service is being undermined by such elementary errors.
